Intake and Assessment

LifeCare offers individual, family or group counseling services, Marriage/couples counseling, nursing and/or psychiatric Medication management, and case management services. Each service is customized to the individual, couple, single parent or traditional family experiencing ongoing issues that need professional help.

Each client’s initial appointment is scheduled with one of our mental health or counseling professionals where the level of care and services are determined.

The following questions will be asked when calling to schedule an Intake and Assessment:

  1. What Services are you seeking (Outpatient Therapy/Medication Management/Case Management
  2. Type of therapist you are seeking (Male/Female, specialty areas, etc.)
  3. Name, address, DOB, phone number and who referred you to LifeCare(how did you hear about us).
  4. Insurance Information (please have your card available), Social Security numbers and other identifying information(drivers license or form of ID, etc.).

Once this information is obtained, the scheduler will give you the date and time of your appointment. Please plan to arrive at least 15min prior to complete paperwork and make copies of your cards.
